I am also an out-of-network provider. You will need to check with your insurance carrier to verify coverage and find out what percentage of reimbursement your plan covers for an out-of-network provider, as well as if you have a deductible to meet before you can obtain reimbursement.

I can prepare a monthly “super bill” for you to submit to your insurance carrier for reimbursement.
